Mossman Hospital is pleased to advise that three Pfizer vaccination clinics will open for youth aged between 12 – 16 years of age.

The clinic will be open at Mossman Hospital, 9 Hospital Street, on Saturday 18 September, Saturday 9 October and Saturday 30 October.

The Clinic hours are 8:30am to 3:30pm. Patients must follow the clinic signs.

Your child will require a consent form which can be downloaded here.

A parent / guardian needs to completes the form and bring it on the day. A hard copy of the consent form will be available at the clinic on the day if you do not have the ability to print one off.

For identification purposes, a Medicare card will be required with the child’s name on it and photo ID of the parent.
